Grants Program
Application Eligibility and Process
Applicants should submit the following:
- Grant cover page (enclosed)
- A letter written together and signed by (1) a pediatrician,
(2) a community organization representative, and (3) a pediatric
resident (if applicable) describing their collaboration
and the roles of each partner.
- Grant application (not to exceed 4 pages)

Grant applications must be submitted by mail or in person
to Lisa Swetz at the address below and must be postmarked
(or received) on or before May 16, 2008.
Faxed or emailed applications will not be accepted.
